Insurance

We offer direct billing and can submit claims to your insurance provider on your behalf. It can reduce your out-of-pocket expenses and eliminate insurance paper work. Our staff can check your eligibility at the office during your visit!

* Direct billing process is based on specific policy, and thus cannot guarantee a patient’s insurance will go through. Please check with provider for coverage.

OHIP Coverage

Patients 19 years old and younger are eligible for an OHIP-insured full eye examination once a year.
Patients 20 to 64 years old with one or more of the following potentially sight-threatening conditions (listed below) are eligible for an OHIP-insured full eye exam once a year.
Patients 65 years old and over with one or more of the following potentially sight-threatening conditions (listed below) are eligible for an OHIP-insured comprehensive eye exam once every 12 months.

Those without any of the following conditions (listed below) are eligible for an OHIP-insured full eye exam once every 18 months.

Potentially sight-threatening conditions:

  1. Diabetes
  2. Glaucoma
  3. Visually significant cataracts
  4. Acute or progressive retinal disease
  5. Acute or progressive corneal disease
  6. Acute optic nerve pathway disease
  7. Active uveitis
  8. Patients taking any of the following medications: Chloroquine, Hydroxychloroquine, Ethambutol, Tamoxifen.
  9. Sudden onset of strabismus (crossed-eyes) due to injury or disease
